Integrative case 03
"Doctor, I can't make it to the bathroom"
Objectives
- Differentiate stress incontinence, urgency incontinence, and mixed incontinence.
- Recognize overactive bladder syndrome as a clinical diagnosis.
- Decide which tests are truly necessary in uncomplicated incontinence.
- Understand that urodynamics is not a routine test.
- Outline an appropriate conservative initial treatment.
- Recognize that symptoms can coexist and change over time.
Stage 1
Initial presentation
Patient: Marta, 62 years old.
Chief complaint: involuntary urine leakage for approximately one year.
Chief complaint: involuntary urine leakage for approximately one year.
"When I feel the urge to urinate, I have to run to the bathroom. Sometimes I don't make it and I leak urine."
She has no fever, hematuria, or flank pain.
What information is most important to obtain now?
Stage 2
History taking
On further questioning, Marta reports:
- she urinates approximately every 1–2 hours;
- she has urgency several times a day;
- she has 3–4 weekly leakage episodes associated with urgency;
- she gets up twice a night to urinate;
- she occasionally leaks a few drops when she sneezes, but this symptom does not bother her;
- she has no difficulty starting urination;
- she feels she empties her bladder completely;
- she has no recurrent urinary tract infections;
- she has had three vaginal deliveries;
- she has no history of neurological disease.
What is the predominant symptom?
Stage 3
Diagnosis
What is the most likely clinical diagnosis?
Key concept
Overactive bladder is a clinical syndrome. It is not synonymous with detrusor overactivity, which is a urodynamic finding.
Stage 4
Studies
Findings
- Physical examination: no relevant findings.
- Urinalysis: normal.
- Postvoid residual: 35 mL.
What simple tool could provide additional information?
Stage 5
Urodynamics
Is urodynamics mandatory before starting treatment?
Pearl for the student
Overactive bladder = clinical diagnosis.
Detrusor overactivity = urodynamic finding.
Stage 6
Treatment
What would be a reasonable initial approach?
Stage 7
Plot twist
Six months later, Marta returns for a visit.
"The urgency has improved quite a bit, but when I exercise, cough, or laugh I still leak urine. That's what bothers me most now."
What changed?

Final result
0/7
What should you take away from this case?
- Leakage with coughing or exercise → stress incontinence.
- Sudden urge that is hard to postpone → urgency.
- Urgency + frequency ± nocturia ± urgency incontinence → overactive bladder syndrome.
- Combination of stress and urgency → mixed incontinence.
- Storage or voiding problems associated with neurological disease → consider neurogenic lower urinary tract dysfunction.
- First characterize the symptom → then identify the likely mechanism → finally select tests and treatment.
